False PHD is a passive, risk-and-reward item introduced in The Binding of Isaac: Repentance. Found primarily in Devil Rooms and Secret Rooms, it completely alters how pills work for the rest of your run.
Instead of turning pills positive like the regular PHD item, False PHD turns all positive pills into negative or neutral ones while providing high-value stat and health compensations. Core Mechanics
Pill Identification: Like the normal PhD, it reveals the identity of all pills before you swallow them, meaning you never have to guess what an unknown pill does.
Stat Down = Damage Up: Whenever you swallow a “Stat Down” pill, your character permanently gains +0.6 Damage. This bonus is retroactive for bad pills taken earlier in the run.
Negative/Neutral Effects = Black Hearts: Taking any pill with a non-stat negative effect (e.g., Addicted!, Amnesia, Paralysis, Bad Trip) will instantly spawn a full Black Heart on the ground.
Pill Conversion: It forces positive pill variants to convert into their negative counterparts (e.g., Tears Up turns into Tears Down, and Gulp! turns into Horf!). Strategic Overview
